What is complicated grief? Recognizing when healing gets stuck

While most people move through grief in their own time, some experience a deeper kind of pain that lingers, disrupts life, and won’t ease — even after many months. This is known as complicated grief. In this article, we explain what it is, why it happens, and how to find help.

Grief is not a timeline — but it can get stuck

Losing someone you love changes everything. For many, grief softens over time, shifting from pain to remembrance. But for some, the sadness remains raw, intense, and disruptive, even long after the loss.

This prolonged, impairing form of grief is called complicated grief, or sometimes prolonged grief disorder.

It’s not weakness. It’s not “grieving wrong.” It’s a sign that grief has become frozen — unable to move forward without support.

What is complicated grief?

Complicated grief is a condition where someone experiences an intense and enduring form of grief that disrupts daily life, functioning, and relationships. It can persist for more than a year, with symptoms that don’t improve — or even worsen — over time.

It’s recognized by mental health professionals as distinct from normal bereavement, and it may require focused treatment.

Signs of complicated grief

While symptoms vary, common features include:

  • Constant longing or yearning for the deceased
  • Intense sorrow that doesn’t ease with time
  • Preoccupation with the person or the circumstances of their death
  • Avoiding reminders of the person — or being unable to let go of them
  • Feeling life has no meaning without them
  • Inability to re-engage with others or daily activities
  • Guilt about “moving on” or surviving
  • Social withdrawal or isolation
  • Depression, anxiety, or even suicidal thoughts

It’s not about how much someone cries — but about whether grief is preventing them from living.

Why does it happen?

Anyone can experience complicated grief, but risk factors include:

  • Sudden or traumatic death (accidents, suicide, violence)
  • Losing a child, partner, or deeply dependent relationship
  • History of depression or trauma
  • Lack of support system
  • Unresolved conflict with the person who died
  • Cultural stigma around grief or mental health

Grief becomes complicated when the mind and body cannot process the loss in a way that allows integration and healing.

How is complicated grief treated?

Treatment often involves:

  • Grief-specific therapy: Such as Complicated Grief Therapy (CGT), which combines elements of cognitive-behavioral therapy with attachment theory.
  • Narrative work: Rebuilding the story of the loss in a way that makes sense.
  • Exposure therapy: Gradual reintroduction to avoided people, places, or memories.
  • Medication: May help if complicated grief is paired with clinical depression or anxiety.
  • Support groups: Reduce isolation and create community among those who “get it.”

Healing doesn’t mean forgetting — it means finding a path forward that honors both love and loss.

Complicated grief and non-traditional goodbyes

When someone chooses cryopreservation, grieving may feel unfamiliar. There may not be a body to bury. There may be lingering hope that they’ll return. These circumstances can complicate emotional closure.

In this case, grief counseling can help:

  • Validate complex, conflicting feelings
  • Support rituals and conversations that create meaning
  • Help families stay connected, even through uncertainty
  • Balance hope with mourning
